Enrolling with Three Rivers Health

Thank you for considering enrolment at our practice. We are inclusive of all peoples and backgrounds and consider everyone is important and unique.

To assist with your enrolment, we have provided an enrolment form for you to download and complete in your own time. If you don’t have the availability to download this please call into our practice or call us and we will mail one out to you.

Complete the form for each person wishing to enrol and return it to our practice. We will process your registration and request your medical file from your previous practice. Young people over 16 years need to sign their own form.

Once you have completed the enrolment form just drop it off at the practice, please remember to take some photo ID to the practice along with the completed enrolment form.

Enrolling with our practice is free. We also provide a free initial half-hour consultation with one of our nurses to best assess your needs and requirements. It’s important that we get to know you and understand your needs and what services we can offer.

Enrolling with our practice gives you access to our skilled clinicians and the following benefits:

Most practices throughout NZ are members of a Primary health Organisation (PHO) who receive funding from the government top provide health services to their communities. By enrolling with our practice, you will benefit from these services and reduced charges. Enrolment is free and voluntary, however if you do not wish to enrol you can still be seen at our practice but the cost will be higher as you will be classed as a casual patient.

The Ministry of Health website (link) has a guide to the eligibility criteria for funding enrolled patients. Eligibility for non New Zealand residents funding is dependent on your work visa status.
